Introduction

Proactive disease management is a proven way to reduce medical complications and lead a full and healthy life. Health Support Systems helps people who are living with Diabetes, Obesity, Hypertension or Heart Disease to better manage their conditions through a combination of self-monitoring, education and qualitative analysis.

Using HSS, you can:

  • Enter and track various health related parameters.
  • Monitor improvements through charts and reports.
  • Track and manage your weight, diet and exercise.
  • Analyze your risk profile.
  • Add your own parameters and "trackers".
  • Record journal entries.
  • Find calorie and nutrient information.
  • Discuss issues at our health forum.
  • Collaborate with your care giver or physician, if they are subscribed to HSS.

Navigation

The application is organized into several sections:

  • Disease Management allows you to enter or edit various health related parameters.
  • Weight Management allows you to enter and track your weight, diet and exercise.
  • Reports & Analysis provides charts, risk analysis and trackers.
  • Diary provides a personalized space to maintain your journals.
  • Forum helps you to discuss health related issues with others who have similar concerns.
  • Customize helps you to add your own parameters or trackers to the application.
  • My account enables you to change passwords or delete accounts.
  • Logout exits you from the current session.

Each section includes several sub-options. While the first level menus are on the left, the second level menus are on the top.

Personal Profile

This page enables you to enter and track your profile information. If you do not know the value for a parameter, you can leave it blank.

From this page you can,

  • Click on Help to know more about a parameter.
  • Click on Save Information to go to the next page.
  • You can also click on the page number to go to that page directly.

Health Monitor

Health Monitor

This page helps you to enter and track your health parameters.

Enter the value and the date on which the parameter was measured (the default date is set to current date). Click on the calendar icon to edit the date.

If you do not know the value for a parameter, leave it blank.

If you skip a parameter, the system will not prompt you for the parameter till the next time it is due. Click View Skipped Parameters if you want to view them before the due date. Based on your profile, the frequency of each parameter's measurement will vary.

If you do not find a particular parameter, you can use the Search option in the screen above.

You can also use the Customize->Custom Entry option to enter values. The custom entry is especially useful if, for example, you want to enter the values of blood sugar for a whole week at one go.

Enter the values for various parameters and click on Save Information to go to the next page.

  • Use this page to edit data that you have already entered.
  • Enter the date range (mm/dd/yyyy) to view the list of parameters for that period.
  • Change the value or the date and then click on Save Information to update.

Search Parameter

Search Parameter

Based on your profile, the frequency of parameter measurement in the Health Monitor may vary.

If you do not find the parameter you were looking for, search for it using this option. To seach for a parameter, enter a space separated list of words (e.g “blood” or “blood glucose”) and click Go.

Weight Summary

Use the weight management option to track your diet, weight & exercise.

Weight Management Summar

The summary page displays information about your body weight, Body Mass Index (BMI), Waist-Hip Ratio (WHR), etc. based on the information you entered in your profile.

If your weight is outside of the normal range, it will also display some information on what you can do to bring it to the ideal range.

Weight Tracker

You can use this page to track your weight.

  • If you are working on losing or gaining weight, the chart will show how you are progressing.
  • You can enter your weight in the health monitor or in the text box on this page.
  • This page will show you a graphical representation of your weight along with your normal ranges.

Exercise & Calorie Burn

.Exercise and Calorie Burn

This page lets you track your exercise and calorie burn.

  • You can find out how much calorie you need and how much you will burn by entering your calorie intake, the amount of calorie you plan to reduce by dieting and the number of calories you plan to burn by various activities.
  • To add a new activity, select the activity under More activities and click on Add.
  • If you do not find your activity, choose the nearest one in terms of strenuousness.
  • If you choose an activity that you perform weekly, specify the daily average here. For example if you bicycle for 2 hours in a week, specify 18 mins (120 mins / 7 days a week) as the daily average for this activity.

Diet

Diet and Calorie Intake

You can use this option to track your diet and your daily calorie intake.

  • If you know the calorie and contents of your diet, enter it and click Save.
  • You can also search for a food item by clicking on the Search link.
  • Using the search option, you can choose your serving size and find out the calorie and contents of a food item.

Food and Calorie Intake

To search for a food type, click on Search and enter the name of the food item or ingredient. Then,

  • Select the result that best matches your request from the displayed list.
  • Classify the type of meal and serving quantity.
  • Click on the Set Details to save this information to the parent form, where you can edit the data and then save it.
  • If you are satisfied with the data, click on Save Record to directly save the data to your diet profile.
  • To delete an entry, click on the delete option next to the displayed item in the main page.

Calorie Finder.

  • Enter the name of a food item to find out its calorific value using the calorie calculator.
  • You can search the database for several thousand food items and ingredients.

  • You can use this page to chart various health parameters.
  • Move the mouse over the [?] symbol in the chart to view the values.
  • If the value is out of range, a 'What can I do about it' link is provided at the bottom of the page to help you know more about the issue.

Tracker

Tracker helps you track a specific set of values on a regular basis. For example, you can track the distance you walk, your carb intake and your blood sugar by adding your own parameters and creating a Tracker profile using the Customize menu.

  • You can also add any information in the Diary and have it show up on the tracker.
  • Pre-defined trackers include Diabetic, Hypertension and Lipid trackers.
  • Select a tracker and enter a date range to view the results.

Risk Profile

Risk Profile

After you fill in your data using the Profile & Health Monitor, use the Risk Profile option to view your risk profile and what contributes to it.

  • Click on Details to find out how something contributes to a risk.
  • Use Help to find out what you can do to reduce the risk.

Personal Diary

Personal Diary

  • Use the personal diary to enter any information you want to track. For example, you can add a note when you change medication, diet or exercise so as to track how it affects your health.
  • The diary entries will also be available on the tracker, making it easier to compare your parameters and diary notes side by side.
  • Click on a date to add or edit the diary entry. Use the arrows to navigate around the calendar. Click Today to go to the current date.
  • Dates that contain some diary entries are marked with “…..

Discussion Forum

Forum

  • You can discuss health related issues at the discussion forum.
  • Click on Start New Discussion to begin a new discussion thread.
  • You can track your discussion and replies at My Discussion.
  • You can view all on-going discussions and reply to them at All Discussions.
  • Delete will remove a discussion entry. This will delete only your entry and not the entire thread or other's replies to it.

  • To start, let us add our own parameter for Carbohydrates and Distance Walked as shown in the screen above.

Custom Profile

This page provides another option for you to enter the parameters and values. This is especially useful if, for example, you want to enter the values for blood glucose for a whole week at one go!

  • Choose your preferred parameters from the droplist.
  • Enter the values and the date.
  • Click Save Information.
  • Please note that no warning message will be displayed even if the parameters are out of range. If you would like data validation, please use the Disease Management->Health Monitor option to enter data.

You can monitor your progress using tools provided in Reports & Analysis.
